Wisconsin Death Certificate Apostille

When you need to apostille a Wisconsin death certificate, you will need to make sure to have a certified copy of the vital record beforehand. If you need to order one, your best bet is to go through the Wisconsin Department of Health Services. Here's what their website says about obtaining one:


Requesting a Vital Record


Wisconsin birth, death, marriage, and divorce records from October 1907 to the present are available from the Wisconsin Vital Records Office. The ordering options and instructions are detailed in the tabs below.

Applications and acceptable identification required: Expired documents will not be accepted. Please include legible photocopies of your identification. Do not mail originals. ONE of the following:

  • State-issued driver's license or ID card

  • U.S. government-issued photo ID

  • U.S. or foreign passport

  • Tribal or military ID card

or TWO of the following:

  • Bank/earnings statement

  • Current, dated, signed lease

  • Health insurance card

  • Utility bill or traffic ticket

  • Vehicle registration/title

Apostille for Wisconsin Death Certificate
