DD Form 214 Apostille

A DD Form 214 is a document issued to military veterans by The Defense Department identifying the veteran's condition of discharge - honorable, general, other than honorable, dishonorable, or bad conduct.

 

The DD Form 214 contains information usually required to verify military service for benefits, retirement, employment, and membership in various veterans’ organizations.

The majority of the time individuals and employers want to see an Honorable Discharge indicated in line 24's Character of Service in the DD Form 214.   1. Mailing or faxing a Request Pertaining to Military Records (Standard Form SF 180) to the National Personnel Records Center (NPRC).

  2. Write a letter to the NPRC regarding your request.

  3. Visiting the NPRC in person regarding your request.

  4. Contact your state or county Veterans agency.
